We will accept notification of a change of name in writing from the guardian/intervenor or solicitor’s acting on their behalf.
- Access to funds scheme
We will accept notification of change of name in writing from the withdrawer or solicitors acting on their behalf.
Evidence to send with notifications
We need to confirm that the notice of change of name is true and accurate. You will need to provide legal confirmation of a change of name, for example:
- A copy of a marriage certificate
- A decree of divorce
- An updated driving licence
- Change of name deed poll
- An updated passport
- A signed declaration, certified by a notary

What happens next?
When we receive your notification and evidence, we will update our records and send you an acknowledgement letter to confirm the change has been made.
We will notify the granter/adult of the change.
We will inform the local authority of the change if welfare powers were granted.
